Notes / Instructions

• ❖ Application must be fully completed. Any data missing from the required fields may result in rejection.
• ❖ If required, a separate sheet can be used to describe your needy situation.
• ❖ Money will not be directly sent to the student; instead, it will be sent to the head of the institution/hostel/book shop, etc. Even for them, no personal accounts are accepted.
• ❖ Applicant’s name should be written with the first name first and last name last.
• ❖ All addresses should be complete to ensure accurate communication.
• ❖ Email ID and phone numbers should be accurate and clearly written.
• ❖ Attestation by the head of the institution is a must.
• ❖ Please use capital letters when filling out the application.

Enclosures

• ❖ Passport size photo (size : 500kb)
• ❖ Attested (by the school) copies of the last two years’ marks sheets
• ❖ Attested (by a gazette officer) of death certificate if incase one or both the parents are not alive
• ❖ Attested (by a gazette officer) of income certificate of parent(s)
